The Notification record is set with Category of "Uncategorized" (screenshot) 2. The Notification user preferneces for the affected user have Notifications for 'Uncategorized' turned off (screenshot) To fix, as affected user, access notification preferences, click advanced preferences, set Uncategorized slide bar to true.
