The out-of-box Business Rule is responsible for populating the chg_model field during record creation. A custom Business Rule was also configured to set the chg_model field during insert operations. Because both Business Rules were attempting to update the same field at nearly the same time, record creation became inconsistent. In some transactions, the flow trigger evaluated the record before the chg_model field was populated. When this occurred: The Standard Change flow trigger conditions were not satisfied.The flow engine skipped execution.Approval records were not generated.The Approvers tab remained empty. Database logs confirmed that affected records were created without the required chg_model value being available when the flow trigger was evaluated. If flow execution issues continue, review Flow Designer trigger conditions and inspect Business Rules that execute during record creation for additional conflicts.
