The attachment count increases with every page load. Reproduced Steps: In Survey Designer, open or create a survey and add an image to the Introduction field of the survey metric type (asmt_metric_type).Send the survey invitation to a non-admin user.Log in as the invited user and open the survey via the OOB Service Portal widget.Note the attachment count on the asmt_assessment_instance record.Close and reopen the survey instance in Service Portal.Observe that a new duplicate attachment is added each time the page is loaded. Upgrade to Australia or later to resolve the behavior. There is no workaround available for versions prior to Australia. Upgrading is the only remediation.
