When upgrades or clones occur, plugin-delivered UI metadata (form sections/elements) can occasionally become inconsistent, which can lead to the Default view appearing broken. Another Root Cause - Instance misses the attachment in the latest record present in "sys_upgrade_manifest" table during the upgrade/cloning due to incorrect clone configuration.
