The issue is caused because the auto sync entity name is true on the entity record, which makes the name field not editable. We introduced this feature in the last release. Before that, you could edit the name field The changes are part of 21.1.x version of GRC apps Pluings: GRC: profiles GRC: risk management / policy and compliance Adding the "auto sync entity name" field to the form and unchecking the field will make the name field editable.
