Steps to Reproduce Open the catalog item in the Service Portal. Submit a request and upload a test file using the attachment field. Submit the request to generate the record (e.g., RITM). Open the created record and review the attachment section. Observe that the uploaded file is not attached to the record and only the file name appears in the description/work notes instead of being available as an attachment.
