This behaviour was working as expected prior to Zurich Patch 4, no custom changes were made to attachment handling, ACLs, or related platform configurations since. The behaviour change is caused by a fix backported to Zurich Patch 4 HotFix 3b. Renaming attachments on 'new unsaved' records is now blocked because the parent record for the attachment doesn't exist in the database yet, and the ACL correctly denies write access when it can't validate the parent. Save the incident first, then rename the attachment. Renaming works normally on saved incident records (and all other records included).2. The alternative would be renaming the file locally, then uploading the file to an unsaved incident or other record.
