It identifies and deletes stale entries that are no longer needed in the new version. This is expected behavior. If you are modifying an item and don't need a question, you can de-activate the question instead of removing it from the catalog item. This way the question does not get deleted and you can include such questions again in the catalog item by using the Insert De-activated questions option.
