This issue occurs if the form layout does not include either the Model [chg_model] or Type [type] fields. Displaying either of these fields on the form prevents the issue.

Reproduced Steps:

Create a Change Request with Type = Emergency . Use the UI Action "Copy Change" . The copied Change Request is created with Type = Normal , regardless of the original type. Display either the chg_model or type field on the Change Request form layout.
