The name of the plugin is "Integration - Multifactor Authentication" plugin (ID: com.snc.integration.multifactor.authentication). Navigate to Plugins from the filter navigator, search for the plugin, and click on the repair option. Verify the 3 options are now available in the "sys_mfa_factor" table. This should fix the issue.
