Steps to Reproduce

Log in as a user Add pipeline setup which has 3 instances. Dev, test, prod Create an app in dev and submit the app request from the dev Admin approves the deployment request and the app is deployed in the instance

Workaround This issue is under review. There is no workaround as the fix needs code changes.
