Questionnaire: Error when you take questionnaire on Platform - Known Error

Questionnaire: Error when you take questionnaire on Platform Description Questionnaire in Utah Patch 0,1,2 & 3 is impacted and being addressed in Utah Patch 4.

Customers who are impacted with it may use the possible work around.

Steps to Reproduce

Summary: When you submit a questionnaire from Platform you see the following error:

There was an unexpected failure with this assessment, invalid type provided

Error from the logs: ErrorMissing assessment: invalidtype: no thrown error

Pre-req: Create a questionnaire to be triggered once the state changes to accepted . This need not be a mandatory questionnaire

Steps to reproduce: 1. Login as admin and create a WOT and assign it to Alex.ray 2. Login as alex.ray and accept the WOT. You should see a questionnaire button. 3. Click on Questionnaire and answers the question and click on Submit.

Actual result: Error Message: There was an unexpected failure with this assessment, invalid type provided

Expected Result: Agent should be able to submit the questionnaire successfully.

Workaround Steps to use workaround:

Login as alex.ray and navigate to "My Assessments and Surveys". Navigate to particular Survey Click on Take Survey button You would now be able to complete the Questionnaire. Once questionnaire is submitted, you would be able to see the success message saying "You have completed this survey".
