URI params should be honoured in the New survey widget Description URI params should be honoured in the New survey widget. This can cause the Assessment instance Trigger ID and Trigger table to be empty.

Steps to Reproduce

1. Log in to the instance as Admin. 2. Open the following in the instance : /sp?id=take_assessment&type_id={sys_id}&sysparm_trigger_table={table}&sysparm_trigger_id={record sys_id}&sysparm_kiosk=true

Expected : The URI params should be honoured by the new widget and a trigger survey instance should be created. Observed : URI params are not honoured by the new widget
