Steps to Reproduce

Steps: 1. log into instance and navigate to view surveys module. 2. Open "Short Customer Satisfaction Survey using Smiley Face" survey and add checkbox signature. 3. Assign the survey to Admin and take survey.

Observed: Image scale options and signature checkbox are not inline in take survey page.

Expected: Image scale options should be inline and signature checkbox & assertion message should be inline in take survey page.
