The "Request Approval" UI Action causes an incomplete Info Message to display on the Form Description Using the "Request Approval" List Choice UI Action causes the "Requested approval for" Information Message to appear on the screen. This message is not correctly populating the Request Item Number as expected.

Steps to Reproduce

1. Open any Request Item 2. Click 'Edit...' in 'Approvers' Related List 3. Add any User 4. Select the Checkbox next to the 'Not Yet Requested' Record in the List View 5. Use the 'Request Approval' List Action 6. See the "Request approval for" Message appears
