Steps to Reproduce

1: Enable NVDA. 2: Open /incident_list.do. 3: Navigate to the 'New' button and open the 'Incident New Record' page. 4: Navigate to the header section > Manage Attachments button and activate it. 5: Navigate to click on the file button and select the file. 6: Navigate to the 'Remove' button and activate it.

Observe the focus moves to the outer of the dialog and the screen reader does not provide the information when activating the 'Remove' button. When the focus moves over the 'Remove' button, NVDA should announce the remove file feature instead of repeating the information for 'Manage Attachments'.
