Cause This partly cause by the current deprecation of Survey Management. the plugin that associate the right event with the notification out o Box isn't pre-installed.

Resolution The survey management domain is not configured correctly, the specific plugin is called "Best Practice - Task Survey Management" .service-now.com/nav_to.do?uri=%2F$allappsmgmt.do" rel="nofollow">https:// .service-now.com/nav_to.do?uri=%2F$allappsmgmt.do

Once you add that plugin in the notification event name will change to task.send_survey. below is a test screenshot of the change in with the plugin added before it was reverted back to current state.
