This can be due to the VA widget being displayed on a particular page as well as by another means such as in New York the Agent Chat configuration (csm portal) or by loading the widget in the portal header/footer which was used to display the Agent across multiple pages in a portalResolutionCheck and remove any duplicate widgets such as checking the portal page itself, reverting the header/footer in the portal theme to OOB and check the Agent Chat portal definition if defined.
