This issue issue has been observed in Chrome (version:75.0.3770.80).Steps to Reproduce 1. Login as administrator using Chrome.2. Go to Assessments > Metric Definition > Types.3. Select the one named "Groups".4. Under the "Related Links", click on "Generate Assessable Records".5. Go back to the record "Groups" from step 3, and click the UI Action "Generate Assessments".6. Once it has finished, go to Assessments > Assessment Instances > Ready to take.7. Within the new assessments generated, copy the user name in the "Assigned to" column.8. Impersonate the user found in step 7.9. Go to Self-Service > My Assessments & Surveys.10. Select the assigned assessment and click on "Take Assessment".11. Scroll down into the assessment record until you see the "Politeness" section.Notice the input field for the question "On a scale of 1 to 10, how respectful was the agent doing the task?" has been filled with a login previously saved on your browser.WorkaroundThis is an issue caused by the browser, not depending on the ServiceNow platform.
