But the survey is visible in the 'assesment_take2' page Release London Cause This issue most probably is caused due to the customization of the script include "SPSurveyAPI" Resolution Starting from London, the "one-click survey" feature has been introduced, which when enabled in the survey definition, it would skip the survey intro page in the portal. In Out of the box, the survey in the portal has two pages "Welcome Page" and "Questions Page". So the widget "Take Survey" will determine which pages should be displayed based on the "one_click_survey" value (data.one_click_survey) and the state value (c.state) defined in the widget's client controller. The Survey's "one_click_survey" value is being pulled into the widget from the script include "SPSurveyAPI". If this script include "SPSurveyAPI" is customized then it would have skipped the London upgrades (which feeds the 'one_click_survey' value to the widgets) and therefore it does not provide the widgets with the information regarding the "one click survey". So the widget code is not able to determine which page to show and therefore it displays the blank page. To resolve the issue, please apply the London upgrades to script include and then apply the customization on top of it, if needed.
