Cause The issue is caused due to the custom widget in the service portal

Resolution Users are using service portal widgets to submit the record producer.

The widget is a custom widget and the widget's submit button allows the users to click on "Submit" button multiple times.

When the "Submit" button is clicked more than once, then there will be orphan records created (along with the original record) with no variable values.

To avoid the issue, disable the Submit button as soon as it is clicked once by the user.
