Custom UI Action is not shown on the Form widget in Service Portal but shows a blue Save button. Issue  Learn why a custom UI Action is not showing in the portal, but shows a blue Save button on the form instead.

Cause When a UI Action has an action name of either sysverb_update or sysverb_insert , it is expected to be the default Save or Submit button. The name is then overwritten and it is presented as a blue Save button.

Resolution This is expected behavior. To change this, you need to change the UI action name to something other than the default sysverb_update or sysverb_insert names since these are used by the system as the primary Save button.
