Log into the instance.Impersonate with any Non-ITIL user.Create an incident.Submit.Impersonate with any admin user.Assign the incident to any user from the Assignment group in the incident.Resolve the incident.Close the incident and survey will be triggered to the caller of the incident.Impersonate back with the caller.Navigate to "My Assessments & Surveys".Click the "Take Survey" button for the survey record available. Survey questions will not be visible to the user.CauseThe Read_Acl_For_Asmt_Assessment_Instance ACL might have been customized and the role ITIL has been added in the requires role tab.ResolutionCheck the Read_Acl_For_Asmt_Assessment_Instance ACL and revert it back to the Out of the box version (OR) modify it accordingly.
