Steps to Reproduce Service Catalog > In service catalog, select any Form Order the catalog item on filling the required mandatory information On Order status, click on the RITM On the Requested Item page, click on Catalog Task - 'New' ui action. You will not be able to update the fields. You will also get a javascript error on top of the Page Once we get the above error, we are unable to perform any UI interaction in the page. Resolution Create the property glide.sc.use_sc_form_v2 and set it to false .

In the instance navigator System Properties > All Properties Create a new property Property name: glide.sc.use_sc_form_v2 Type: true | false Value: false
