When using a personalized form icon, icons appear beside the Configuration Item field


When using the Personalize Form list, icons appear on either side of Configuration Item field.

Steps to Reproduce

  1. Navigate to Change > Create new and create a new normal change.

  2. Select the Personalize Form drop-down list in the form header and deselect the Configuration Item field.

    Note that the field is now hidden.

  3. In the Personalize Form drop-down list, select the Configuration Item field.

    Note that icons not visible previously are now visible.
