Template bar is not showing by default.
In the filter navigator, type incident.do.
Note that at the bottom of the new form, the template bar does not appear. It also does not appear if you use the Toggle template bar button in the form header to show it.
After carefully considering the severity and frequency of the issue, and risk of attempting a fix, it has been decided to not address this issue in any current or future releases.
