When creating a new record in a reference field, reference fields on the new record form do not display the lookup icon.
Navigate to Incident > Open and select any incident.
Click the lookup icon next to the Assigned to field.
In the Users dialog box, click New.
Right-click in the form header and choose Configure > Form Layout.
Add any reference field to the form (for example, Domain) and click Save.
Note that the reference field is there but the reference icon is not displayed.
