Looking at the subject lines from your attachments and the records, these are the two notifications that are being sent. The notification that references the sc_request table, has been deactivated at 2018-02-23 04:43:45 by a user and this should solve your issue. The reason they were both being sent was that the conditions were matching for each notification. The sc_request was being closed and a new assessment instance was being created.
